The Smart Meds program at UF Health addresses serious issues that can be caused by polypharmacy and medication-related errors among seniors. It is estimated that 30% of seniors use eight or more medications daily. Nationwide, over 175,000 seniors annually will have an emergency room visit caused by a commonly prescribed medication. Our Smart Meds program hopes to decrease these avoidable complications and ER visits by providing education to seniors about their medications and supplements. During these programs, a multidisciplinary team including pharmacists, nurses, and students review medications with the seniors. Our team is able to identify any potential drug-drug interactions or misinterpretations of medications regimen leading to improved patient safety and health outcomes.
